The following forms are available for download in PDF Format. Each form
has a brief description to provide additional information regarding the
purpose of the form.
Crop Certification
Form 50 –
Report on Plot Production
This form must be completed for all Select and Foundation seed production
plots. It provides feedback to plant breeders on the
adaptability of varieties to the region the plot is being produced.
Form 101 – Land Use Verification – Prior to Planting
If you are uncertain of the eligibility of land for pedigreed seed crop
production, then this form may be completed and submitted to the CSGA
office prior to planting. CSGA will review the land use plan and send
you confirmation on whether your Plan meets CSGA’s requirements.
Form 154 – Application to Commence Probation Plot Production
This form is to be completed by growers wishing to begin probationary
work towards becoming a Select seed plot grower or a Foundation seed plot
grower of canola, hemp, mustard, oilseed radish, rapeseed,
safflower or sunflower.
Form 179A –
Authorization to Assign Crop Certificates
This form may be used to assign crop certificates of annual crops or to
assign crop certificates of perennial crops for one year at a time to
another grower or seed company.
Form 180 – Declaration of Percent Hybrid Seed
This form is to be completed for each hybrid seed production field. It
is used to declare the percent hybridity of the resulting seed crop.
Hybrid seed production fields must meet minimum hybridity standards.
Form 182 - Refuge Declaration
This form is to complete certification of pest tolerant pedigreed seed
crops with variety descriptions that include this additional
certification requirement.
Form 200 – Appeal
This form is to be completed by growers who want to appeal the CSGA’s
decision to demote or decline pedigreed status of a seed
crop. This form along with supporting documents is presented
to the Appeals Committee of the National Board during their November
deliberations.
Form 43A –
Application for Plant Breeder Recognition
In order to produce Breeder seed, plant breeders must first become
recognized by the CSGA. This is an application to become a
CSGA-recognized plant breeder, an associate plant breeder or a variety maintainer.
Form 45 – Demotion of Breeder Seed
For plant breeders to demote a CSGA Breeder seed crop certificate to a
Foundation, Registered or Certified status crop certificate, this form
must be completed and submitted along with a POV seed sample to the
CSGA.
Breeder Quality Management System (QMS) – Checklist
For plant breeders developing their quality management systems, this
checklist provides a summary of the elements that plant breeder
consultants recommended for Breeder seed production quality manuals.
